Thank you for allowing us to participate in your patients’s care. To book appointments for your patients, please fill out the referral form below and fax to 317-214-6015. Our staff will call your patients to schedule their appointments and we will also inform you of their appointment time and date.
Please fax or mail to us any relevant health records that you have for your patient so that Dr. Tran can review them prior to the appointment.

Thank you for trusting us to care for you. To make an appointment with us please ask your family physician or primary care physician to fill out the referral form above and fax to us at 317-214-6015. You may also need to sign an Authorization form for the release of your medical records to Dr. Tran.
If you don’t have a primary care physician and your insurance plan allows you to make an appointment directly to be seen by a specialist, you can call us at 317-214-6020 to schedule an appointment. Our staff will need to ask you a few questions and verify your insurance in order to book an appointment for you.
If you don’t have medical insurance and are willing to pay for your visit you can call us at the above number to schedule an appointment. Payment will be expected at the end of your visit.
We will mail to you a packet with forms to fill out before you come to your first appointment. Please be sure to arrive at least 15 minutes before your scheduled time as you will be asked to answer some questions about your current health status prior to being seen by Dr. Tran.
